I now that this has been posted before, but I can not find the link. I am looking for a sheet ( maybe in word format) to rate my cigars that I have tried. I know I saw one previous that had a space to paste in the wrapper for easy identification. Any help would be greatly appreciated.

I have a dosier or how ever you spell it... Ya know, a book that you take notes on all the stogies you smoked and a place for the label and all. I only used the first page. You want it?
