Actually, it's quite easy. First, you take a draw on your favorite lit cigar. Then you open your mouth like this. Then you go like this. But be certain not to confuse "this" with "That". Doing that will hurt your larynx and just frustrate you.

I do it a different way but this is how it was explained to me.

1) Take a draw from your cigar
2) Make an O shape with your mouth
3) Curl your tongue, kind of like you are trying to touch it to the back of your throat but not really that far
4) flick you tongue to the front of your mouth thus pushing the smoke out
