If he knowingly "double-dipped", F him.
If it was staff misdeed or gross sloppiness, they need to be fired.
If it was just true sloppiness on HIS part, it may change some peoples' view of his competency, but I doubt it.
If it truly was an innocent mistake or series thereof, who cares?
