If the original PDF was created from a scan or raster image, printing to a new PDF with comments over top may work visually. But when editing with Acrobat's tools, the original unretouched image will still be there. If the original PDF has the fonts and ascii text embedded within the document, printing to a new PDF again may work visually, be the text will still be embedded, and most likely be searchable. Simply Saving with comments over top an image or text will not "remove" the original content of he PDF. It's a "Comments" feature, not an eraser or paint bucket.
