Many of my Spanish speaking clients used to refer to me as "Doctor Fenster" as opposed to "Abogado Fenster," which translates directly to "Lawyer Fenster." According to a former lawyer office-mate who is Columbian, this is common in certain Latin American communities.
Yet, even though I have a doctorate degree, it just personally never felt right to me. But, as to Jill Biden's desire to be referred to as "Dr. Biden," So what? Who cares? She is part of the academic world where this is customary.
So, what ever floats her boat. She has earned it.
