I'm assuming everyone's panic is because people will see naughty bits of other people which they find attractive. If that's the case, then locker rooms shouldn't be based on the individual's gender at all. They should be based on what gender you find attractive. People who want to screw men, go in one locker room, people who want to screw women go in another locker room and people who like screwing either sex get to have lots of fun in their own locker room.

Alternatively, if you want to look at locker rooms as functional areas which are slightly specialized to match the gender of those using it (ie, tampon dispensers/urinals that sort of specialization) then they should be based on sexual phenotype (#2 above)

If you look at locker rooms as functional areas which are slightly specialized to match the needs of that gender's clothing (places to hang skirts etc) then it should be based on sexual identity (#3 above).

There isn't a good answer to this problem. But in all reality, there isn't a BAD answer either. What's the worst thing that happens if someone goes into whatever locker-room they're most comfortable with? They get changed where they're comfortable, then leave. If your comfort in a locker-room is based on who's around you, you probably have some problems of your own.

Truth be told, it shouldn't matter. Most 14 year old boys won't do anything to a 12 year old girl.
